PRESCRIPTION REFILLS

Please provide the following information:

Patient name
Date of birth
Phone number
Medication name, strenth, quantity and how it is taken
Pharmacy name, location and phone number

For all prescriptions you must be seen within 12 months or per Dr discussion
all rx prior authorizations take 1 week to process
Please allow 48 hours for prescription refills.

  • Medical Mutual (except HMO or Advantage Choice)
  • UHC (except Navigate, Core, Compass, Charter, Choice HMO, Select HMO)
We do not accept Medicaid, CareSource, Tricare, Government Health Care Programs Plans or any new Medicare patients.

If during the course of treatment, your benefits change, the financially responsible party is accountable for ALL changes. Please let us know of any changes in your health insurance, so we can deal with it in a timely and accurate manner.

REFERRALS


Referral to specialists are an important part of the health care process. We at Summerhill Family Medicine have an extensive network of quality referral physicians. When a referral is initiated through our office we verify network physicians and complete the needed prior authorizations.
